Workers Compensation

With over 40 years of combined experience, the physicians of PMPSF are dedicated to assisting the injured worker return to pre-injury status. We coordinate with case managers and adjusters to rapidly assess and offer treatments protocols to injured workers. We offer IME and EMA evaluation as well as second opinions. The physicians see the patients directly and do not use physician extenders to make clinical decisions.

Initial Evaluation Form

You may download and print the Initial Evaluation Form; then, complete and fax to our office at 954-974-2335. We will contact you immediately upon receipt of the faxed Initial Evaluation Form and schedule your injured worker.
